Short answer: treat the number on your IRCTC screen as the only price that counts, and read the conditions shown next to it before you pay. Premium Tatkal is not just Tatkal with a different label. It is a separate quota, and its fare is not fixed in advance.
How it differs from Tatkal. IRCTC's Premium Tatkal quota uses dynamic fare pricing: the fare component may increase with subsequent bookings. Press coverage of the scheme describes it as costlier than Tatkal for that reason. Standard Tatkal has a fixed extra charge on top of the base fare, so its price is easier to predict.
Same opening routine. Summaries of the scheme state that the advance reservation period for Premium Tatkal is the same as for Tatkal, and that booking opens on or after 10:00 on the opening day. Check the opening time for your class on IRCTC before the day, and follow the usual Tatkal preparation, such as saved passenger details, so you are not typing under time pressure.
Compare two things before paying. First, the exact total fare shown for Premium Tatkal against the standard Tatkal fare for the same train and class, if standard Tatkal still has seats or a waiting list you are willing to accept. Second, the conditions on the quote itself: whether the quota gives a confirmed seat or a waiting-list option, and what the cancellation and refund terms say.
Do not rely on old articles for the numbers. Rates, caps and refund rules for these quotas have been revised over the years, and articles that quote a formula or a refund rule may be out of date. We are not giving a price multiplier or refund promise here for that reason. The screen that asks for your payment is the current source.
A quick decision test. If the live Premium Tatkal fare is acceptable even if you never get a refund, and the date cannot move, it may be worth it. If your plans could change, or a cheaper alternative exists on another train or day, the extra cost is a larger risk.
